Effective Date of this Policy: January 1, 2026
These are separate from the menu and can be used on a membership enrollment form or agreement.
1. Six-Month Commitment: All memberships require an initial six-month minimum commitment.
2. Automatic Renewal: Memberships automatically renew monthly following the initial six-month term.
3. Payment Method: A valid credit or debit card must remain on file for recurring monthly membership payments.
4. Non-Transferable: Membership benefits are exclusive to the enrolled member and cannot be transferred or shared.
5. Monthly Service Benefits: Each membership includes one facial and one eligible enhancement per billing cycle, based on the selected tier.
6. No Rollovers: Unused monthly services and enhancements do not roll over into future months. Any unused benefits expire at the end of the billing cycle.
7. Membership Pause: Members may pause their membership one time. The duration and notice requirements for this pause still need to be established.
8. Membership Discounts: Additional treatment and skincare discounts are valid only while the membership is active and in good standing.
9. Promotional Offers: Membership discounts cannot be combined with other promotions, specials, or discounted offers.
10. Appointment Cancellation Policy: A minimum of 24 hours' notice is required to cancel or reschedule an appointment.
11. Late Cancellations & No-Shows: Missed appointments or cancellations made with less than 24 hours' notice may result in forfeiture of the included monthly service.
12. Treatment Eligibility: All treatments and enhancements are subject to provider approval and individual eligibility.
13. Birthday Reward: Active members receive a $50 treatment credit during their birthday month. The credit must be redeemed within that month.
